Alternatives (for the Older Adult, Inc.
) is a 501(c)(3}, private, non-profit organization serving sixteen counties in Midwestern Illinois.
Since 1984, it has been the mission of Alternatives to promote the independence and quality of life for older adults, adults with disabilities and their families.
Alternatives' professionals are connecting with ONE IN FIVE individuals over the age of 60 in a sixteen-county region.
*BASIC PURPOSE/SCOPE*
Provide confidential assistance and planning to help individuals thrive in their caregiver role.
The Caregiver Counselor will provide counseling, education, information, and assistance to caregivers of older adults and older adults raising children.
Requires some travel throughout the ten counties this program serves.
This is a 30 hour a week position with benefits.
*PRINCIPAL RESPONSIBILITIES:*
*Implementation of the Caregiver Program*
Provide individual support to caregivers.
Complete person-centered assessments and assist caregivers in establishing realistic and appropriate plans of care.
Mediate meetings with the family, caregiver, and/or care recipient.
Facilitate peer support meetings for caregivers and older adults raising children.
Services may be provided face-to-face, virtually, or by telephone.
*Public Education and Awareness*
Provide community education and awareness about caregiver issues through educational programs, special events, health fairs, and presentations.
Plan and develop new workshop materials for caregivers.
Develop content for quarterly program newsletters.
*Documentation*
Provide accurate and timely documentation of work in computer-based applications and digital files for program and grant reporting.
Maintain quality assurance of caregiver files through peer review.
*Networking*.
Responsible for coordination of internal staff and external program partners to maximize caregiver support.
Network with local service providers to become familiar with resources, build collaborations, and promote Alternatives’ services.
*Program Development*
Work with the Program Director to develop, improve, and streamline services to ensure sustainability.
Offer suggestions for program improvements and actively participate in the advancement of the program.
Conduct satisfaction surveys and provide outcome measures.
Assist with submission of grants.
*Other Duties as Assigned*
Ensure smooth operations of services and to complete other tasks as assigned in support of the organization’s mission.
*REQUIREMENTS:*
Bachelor’s degree in Social Work, Counseling, or Psychology required.
Master’s degree preferred.
Experience working with family systems and/or geriatric experience recommended.
Applicant must have strong interpersonal skills and work from a person-centered perspective.
*SUPERVISION: *Position is responsible to the Program Director.
