*Job Summary: *
A Registered Nurse administers skilled nursing care to patients intermittently in their place of residence.
Physician orders and care plans are performed under the direction and supervision of the Director of Nursing/Clinical Manager.
*Responsibilities: *
* Provides services by the plan of care.
* Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs.
* Initiates the plan of care and necessary revisions.
* Provides those services requiring substantial nursing skills.
* Initiates appropriate preventive and rehabilitative nursing procedures.
* As per Agency policy, prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a family manner.
* Coordinates services.
* Informs personnel of changes in the condition and needs of the patient.
* Counsels the patient, family/significant others in meeting nursing and related needs.
* Participates in and presents in-service programs.
* Understands and adheres to established Agency policies and procedures.
* Processes orders and notifies physicians of patient needs and changes in condition.
* Completes certification/recertification orders and discharge summaries.
* Participates in peer review, Quality Assessment, and Performance Improvement as needed.
* Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition by defined time frames.
* Determines the amount and type of nursing needed by each patient.
*Qualifications:*
* Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
* Two (2) years of nursing experience in a home health setting is preferred.
* One (1) year of experience with visiting patients is preferred.
* Acceptance of the philosophy and goals of this Agency.
* Ability to exercise initiative and independent judgment.
* Ability to participate in physical activity, if necessary.
* Ability to visit neighboring suburbs from Yorkville like Oswego, Aurora, Naperville, and Plainfield when needed.
*Working Environment:*
* Works indoors in Agency office and patient homes and travels to/from patient homes.

* High risk.
